We provide one on one support for all patients who have had a heart attack, heart surgery, or who have heart disease.
Our 12-week programme includes advice and support on:
- Risk factors and heart health
- Stress management
- Nutrition and exercise
This programme can be delivered in your home, marae or community setting.
We also provide health screening for all persons who may be at risk of cardiovascular (heart) disease.

Who is this service for?
Those who have had a hospital admission for acute coronary syndrome (unstable angina or MI - myocardial infarction) coronary surgery, coronary by-graft, angioplasty or angiograms;
Those who are being treated for coronary heart disease in primary health care;
Initial cardiac screening assessments for specifically Maori male from 35 years and over, Maori female from 45 years and over (but not exclusive to this particular group).
