We will help you to:

• Understand your chronic conditions
• Understand your medications
• Develop individual care plans with whanau involvement
• Access health programmes and services which will assist you in achieving your goals

Who is this service for?

Access to this service is managed on clients chronic care conditions and priority of need and capacity to benefit. This service is aimed at those clients and whanau who require information and support to effectively manage their chronic condition.

Potential clients will be enrolled, assessed and prioritised as to the extent of the service required. If clients currently using the service are seen to be managing their chronic conditions effectively they will receive minimal follow-ups or get discharged to their GP services.
