Te Hiku Hauora has been providing essential home support services to whanau in our community for over 20 years. Home support services enables our elderly and disabled whanau members to continue live independently in their own homes. The ability to continue to live within an environment that our loved ones have been familiar with over many years is beneficial for their health and well-being. Home support services may include, for example, help with showering, dressing, cleaning, laundry or meal preparation and assistance with attending appointments and shopping (if included on referral).
Accessing home support services is a simple process which starts with a needs assessment being carried out in your home by an Assessor from the Needs Assessment Service. Referral to this service is generally through your Doctor; who is able to address your eligibility for support and refer according to your specific needs.
The needs Assessor will make a time to meet with your elderly whanau member, carers and/or whanau support persons to identify what your loved one may have difficulty doing and prioritise their needs.
Once the Assessment has been completed; usually within a short time frame, we will receive a referral from the Assessor setting forth the specific care requirements for your whanau member and the number of support hours required each week. We will then make contact with your whanau member (or their assigned contact person) and during that discussion make arrangements for the days and times for care to be provided by our support worker.
Our support workers are trained to a national standard, hold current First Aid certificates and sign a privacy and confidentiality agreement; with many of them having provided care and support to whanau in their own homes for a number of years.
